The salary range for this position is CAD $41.42/Hr. - CAD $55.91/Hr.

The Community Health Nurse provides comprehensive nursing care to youth with complex health needs by assessing, planning, implementing, and evaluating individualized care plans. Provides health education, supports transitions of care, connects clients with community resources, and collaborates with the interdisciplinary team while following BCCNM standards, PHC policies, and established clinical protocols.

Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care. Education, Training and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) plus two (2) years’ recent, related experience working with youth and young adults in a primary care and mental health/substance use setting; completion of the Point of Care HIV Testing course[AM1.1]; and successful completion of the BCCDC Immunization certificate. BCCNM Certified practiced in Reproductive Health (Sexually Transmitted Infections (STI)) or evidence of enrollment in required certified practice courses as specified by the BCCNM.


Skills and Abilities

• Knowledge of and exposure to trauma-informed care, violence prevention, gender-affirming care, harm reduction, recovery-oriented practice and indigenous cultural safety training.
• Demonstrated knowledge of wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy and pelvic exams, diabetes care, pain management, STI screening, testing, diagnosing and treatment within scope of nursing certified STI practice, and pregnancy and urine drug tests.
• Demonstrated knowledge of client/patient-centered education principles, methods and tools as it relates to healthcare needs including pain, addiction, diabetes, wound care.
• Ability to role model and employ effective conflict resolution, de-escalation and reconciliation approaches and techniques.
• Ability to communicate effectively both verbally and in writing.
• Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
• Ability to work as a member in an interdisciplinary team.

Your Day to Day:

• Assesses clients using charts, interviews, observations, questionnaires, and interdisciplinary input.
• Develops, prioritizes, implements, and evaluates individualized nursing care plans.
• Provides community outreach and primary care nursing services.
• Performs wound care, addiction assessments, harm reduction, HIV/HCV testing, phlebotomy, and pelvic exams.
• Provides diabetes care, pain management, STI screening/treatment, pregnancy testing, and urine drug testing.
• Monitors client progress, reassesses needs, and adjusts care plans to support successful transitions.
• Responds to changes in client condition, including crisis and life-threatening situations.
• Documents assessments, interventions, client progress, and transition/discharge plans accurately.
• Provides education, clinical guidance, and support to clients, families, nursing staff, and interdisciplinary teams.
• Participates in quality improvement, research, policy development, education, and advocacy to promote client rights and quality care.
