Summary
Provide compassionate, hands-on care as a Licensed Practical Nurse in a collaborative, patient-centered environment. In this role, you’ll work alongside an interdisciplinary team to deliver high-quality nursing care, support residents’ physical and emotional well-being, and contribute to care planning, assessments, and ongoing evaluations. You’ll practice with autonomy while having the support of RNs when needed, ensuring safe and effective care. Your Day to Day:
Delivers safe, respectful nursing care while prioritizing individualized care plans.
Performs comprehensive assessments and reports abnormal findings to RN or team members.
Communicates with physicians and carries out and processes medical orders accurately.
Documents all care, treatments, and patient progress in appropriate systems.
Assists with patient admissions, transfers, and preparation for procedures.
Administers medications (excluding IV) and monitors patient responses.
Responds to emergencies and reports incidents according to protocols.
Supports patient and family education, encouraging participation in care decisions.
Collaborates with the interdisciplinary team on care planning and discharge preparation.
Assists with staff orientation, participates in meetings, and performs basic clerical and equipment-related duties.
