Providence Health Care is now hiring a Patient Care Manager to support the opening of the next phase in Road to Recovery's (R2R) continuum of person-centered and compassion-driven services: a ~50 bed off-site treatment center. 

In this role, you’ll play a pivotal role in shaping how this program comes to life—leading planning, designing care delivery, and ultimately overseeing a high-performing interdisciplinary team supporting clients through treatment and recovery. This is a rare opportunity to combine program design, operational leadership, and meaningful impact in one role—helping create a service that meets people where they are and supports long-term recovery.

Road to Recovery

The R2R initiative strives to better address treatment of substance use issues by providing the right support to each client at the right time. People who use substances deserve the same access to evidence-based care as anyone else, free from stigma. As with all of our services at Providence Health Care, R2R delivers care in a way that strives to be person-centred, trauma-informed and culturally safe. Launched in 2023, the R2R supports clients at every phase of their wellness and recovery journey by coordinating seamless care, even if there are setbacks or relapses along the way.

The R2R continuum of services in operation today includes 25 detox beds located at St. Paul's Hospital, 20 transitional care beds for clients awaiting space in a treatment program or long-term housing, and the Aftercare Team, which launched in Fall 2024, providing outreach aftercare in the community.

Your duties will include:

• Lead the planning and launch of a new treatment program: Play a key role in shaping a new substance use treatment facility—working with clinical and project leaders to design care models, workflows, staffing structures, and operational processes that support safe, effective, and recovery-oriented care.
• Oversee day-to-day operations and team leadership (post-launch): Provide operational leadership for an interdisciplinary team, ensuring high-quality, patient-centred care delivery while managing staffing, budgets, and overall program performance.
• Drive quality improvement and service excellence: Use data, quality improvement, and risk management strategies to continuously evaluate and enhance care delivery—ensuring services are efficient, evidence-informed, and responsive to the needs of clients and families.
• Build strong partnerships across the care continuum: Collaborate with internal teams and community partners to support seamless transitions in care, strengthen connections to treatment and recovery services, and ensure clients are supported throughout their journey.

Qualifications:

• Masters degree in a related Health Profession plus a minimum of five (5) years’ experience at the middle management level in a medium to large health care organization or an equivalent combination of education and experience.
• Community substance use services experience preferred, especially treatment/recovery services
• Clinical service design and planning experience an asset

• Competitive Pay: $118,923 - $170,951 CAD per year, depending on experience

Your Day to Day:

• Provides leadership to the interdisciplinary team including CNL(s) and/or Unit Supervisor(s) and/or equivalent nursing leadership roles to ensure patient /resident/family focused care is delivered within a coordinated and integrated care delivery model.
• Collaborates with the Physician Operations Leader to plan and manage the provision of effective and efficient care within assigned sub-units to support the achievement of operational and strategic objectives of the clinical program and consistent with the mission, vision and values of PHC.
• Collaborates with other Leaders to lead and/or participate in process improvements by engaging the interdisciplinary team in problem-solving and applying tools of quality improvement, utilization and risk management including implementation of standards and tracking mechanisms and use of data and variance analysis.
• Works with community and/or internal partners as appropriate to ensure the provision of timely care to meet the needs of patients/residents/families. Assists team to manage risks, incidents, family/client complaints and discharge planning.
• Ensures effective, two-way communications with and among team members so that each team is aware of and understands changes impacting them, the site, the organization and how such changes will impact delivery of services.
• Provides the interface between the assigned sub-units and corporate /support systems to identify needs and participate in identifying solutions that will enable the front-line delivery of care.
• Collaborates with physician partner to develop the operating and capital budget for the areas of responsibility, monitor productivity indicators, budget variances and analysis. Approves timekeeping records, authorizes overtime and compiles periodic operational reports.
• Develops, recommends, and implements, in collaboration with Human Resource Representative(s), strategies for recruitment and retention of staff that support the goals and objectives of the clinical area(s) of responsibility.
• Facilitates the development and implementation of orientation programs for new staff to provide the appropriate education in clinical practice routines, and hospital policies and procedures.
• Develops, recommends, implements, and evaluates staffing patterns, rotations and vacation schedules to ensure sufficient staff to meet patient/resident care needs.
• Participates as a member of the leadership team by performing related duties such as formulating employer demands for collective bargaining, developing strike contingency plans, representing the employer during grievance proceedings, participating in the managerial 24 hour call structure and facilitating and initiating service delivery change proposals.
• Creates an environment that embraces and facilitates interdisciplinary team learning, evaluation and continuous improvement.
• In collaboration with Professional Practice Leaders and Advanced Practice Nurses:
  • Ensures that team practices align with established clinical/corporate standards, guidelines, systems.
  • Determines skill mix and staff complement required to support clinical care in assigned sub-units; selects staff; determines performance expectations and provides coaching based on the goals and objectives of the clinical areas of responsibility.
  • Facilitates the development and implementation of orientation programs for new staff to provide the appropriate education in clinical practice routines and hospital policies and procedures.
  • Develops, recommends, implements and evaluates staffing patterns, rotations and vacation schedules to ensure staffing to meet patient/resident care needs.
  • Jointly conducts performance appraisals of interdisciplinary care teams and their members.
  • Ensures fieldwork opportunity for interdisciplinary students including orientation to the work place and evaluation /feedback of performance.
  • Fosters the concept of evidence-based practice.
• Performs other related functions as assigned.
