Your Day to Day:
- Coaches staff in clinical nursing skills and behaviours by role modeling and through individual and group teaching.
- Assesses, plans, implements, and evaluates nursing education programs within the clinical area in collaboration with the Patient Care Manager, Director of Professional Practice and Nursing, other nursing staff and other health care team members. Assesses requests for programs, validates learning needs through needs assessment and consultation, develops proposals and new programs as required to meet learning needs, coordinates, and schedules educational activities, identifies outcome indicators, and evaluates and revises programs accordingly.
- Assesses patients/families physical, emotional, and mental readiness and ability to learn, through physical assessments, discussions, completion of questionnaires, chart review and through collaboration with other members of the interdisciplinary team.
- Organizes resources, supplies and equipment for patient/family home as required. Develops educational materials for patient/family such as pamphlets, brochures, instruction sheets, and manuals.
- Teaches patient and or significant others(s) to administer treatments and operate equipment safely, both at home and in the hospital.
- Coaches patient/family to develop and maintain health promoting behaviours within the context of the illness or disease and advocates for patient/family when they are unable to do so for themselves.
- Monitors patient progress by adjusting plan of care to ensure optimum patient outcomes, maintaining contact with patient at home and/or liaising with outside agencies and linking patient with community-based resources or referral options, as required. Maintains and updates patient treatment records, statistics, and other necessary documentation. Participates in interdisciplinary rounds and provides input regarding patient/family progress.
- Identifies trends in patient outcomes and make recommendations to the Patient Care Manager and/or Director of Professional Practice and Nursing on program modifications. Develops, implements, and evaluates patient education programs.
- Participates in research activities and the development of quality improvement initiatives with program/service by as promoting the use of research findings into practice and incorporating information into policies, procedures, reference materials, educational materials and teaching sessions. Acts on recommendations from interdisciplinary team regarding changes/modifications to educational materials, handouts, and in-service sessions.
- Supports nursing practice across PHC and represents the program/service by taking part in nursing, hospital/organization wide and community committees, designing, implementing, and evaluating hospital/organization wide educational programs, liaising with external agencies, and reporting information back to the program/service. Participates as a member of a teaching group. Teaches other members of the interdisciplinary team as required and acts as a clinical consultant to internal and external health care professionals in area of expertise.
- Provides direct nursing care, including physical assessments and administering medications and treatments.
- Participates as requested by the Patient Care Manager and/or staff member in performance planning and coaching by providing input into staff development, preparing, and presenting educational materials and in-service sessions related to program/service or area of expertise.
- Reviews program policies and procedures and makes recommendations to the Patient Care Manager and/or Director of Professional Practice and Nursing for changes. Updates as approved.
- Monitors, evaluates, and makes recommendations to the Patient Care Manager regarding physical space, equipment, supplies and staffing requirements.
- Performs other related duties as assigned.
