As a Licensed Practical Nurse (LPN) at Providence Health Care, you’ll provide compassionate, person-centered care to patients or residents in collaboration with an interdisciplinary team. Reporting to the Care Manager, you’ll assess, plan, implement, and evaluate care, support patients’ physical, emotional, and spiritual well-being, and advocate for their needs. Working within your scope of practice, you’ll also contribute to care plans, participate in care conferences, mentor new staff, and ensure safe, high-quality care aligned with PHC’s mission and values.
