The salary range for this position is CAD $41.42/Hr. - CAD $55.91/Hr.

There’s never been a better time to join the Hospital at Home team. We’ve recently launched a new partnership with the Community Transition Care Team (CTCT)—a collaboration that expands access to acute care for residents of the Downtown Eastside (DTES) who are unhoused or precariously housed.

This initiative enables vulnerable individuals to complete complex medical treatments—such as extended IV antibiotic therapy—within CTCT’s supportive, trauma-informed environment, while continuing to receive housing, mental health, and substance use supports. Together, we deliver safe, integrated, and equitable care to those who need it most.

Your Day-to-Day

  • Deliver daily in-person nursing care to patients receiving hospital-level treatment at home
  • Monitor vital signs remotely (e.g., blood pressure, heart rate, oxygen levels)
  • Administer medications, IV therapy, wound care, and oxygen therapy
  • Collaborate with physicians, social workers, pharmacists, and allied health professionals
  • Educate patients and families on treatment plans and self-care
  • Respond to patient needs and escalate care when necessary

What You Bring

Education

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • Minimum of one year of acute care nursing experience
  • Valid BC driver’s license and access to a personal vehicle for local travel

Skills and Abilities

  • Ability to communicate effectively both verbally and in writing.
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Ability to work as a member in an interdisciplinary team.
  • Ability to apply decision-making and analytical skills.
  • Ability to deal with others effectively.
  • Ability to role model conflict resolution strategies.
  • Physical ability to carry out the duties of the position.
  • Ability to prioritize and organize work.
  • Ability to operate related equipment.
  • Ability to teach and identify patient/client learning needs.
  • Ability to lead patient/family group sessions.
