Summary

Do you want to help redefine the future of acute care in British Columbia? Providence Health Care, in collaboration with the BC Ministry of Health, is leading the way with Hospital at Home —a groundbreaking model that delivers hospital-level care in the comfort of patients’ homes.

As an RN/RPN in this innovative program, you’ll be part of a passionate, interdisciplinary team that’s transforming how care is delivered. You’ll support patients with acute medical needs who would otherwise be admitted to hospital, helping them heal in familiar surroundings while maintaining safety, dignity, and autonomy.

“The interdisciplinary team is incredibly strong, and I’m proud to be part of it. We’re able to build stronger relationships because we’re in the patient’s home environment. It’s a collaborative patient-provider partnership that feels different from other care settings.”— Chloe, RN, Hospital at Home

There’s never been a better time to join the Hospital at Home team. We’ve recently launched a new partnership with the Community Transition Care Team (CTCT)—a collaboration that expands access to acute care for residents of the Downtown Eastside (DTES) who are unhoused or precariously housed.

This initiative enables vulnerable individuals to complete complex medical treatments—such as extended IV antibiotic therapy—within CTCT’s supportive, trauma-informed environment, while continuing to receive housing, mental health, and substance use supports. Together, we deliver safe, integrated, and equitable care to those who need it most.

The Hospital at Home program, based at St. Paul’s Hospital (SPH), is a provincial initiative that delivers acute-level care to eligible patients within a 15-minute driving radius of SPH. It serves patients with acute medical conditions who are clinically stable enough to be safely cared for at home.

Within the context of a client- and family-centered model of care, and guided by Providence Health Care’s Mission, Vision, Values, and strategic directions, the Registered Nurse, Hospital at Home:

  • Promotes a safe, respectful, and inclusive environment for patients, families, visitors, and staff
  • Assesses, plans, implements, and evaluates nursing care for patients with acute health needs in the home and/or hospital setting
