Summary
The Hospital at Home program, based at St. Paul’s Hospital (SPH), is a provincial initiative that delivers acute-level care to eligible patients within a 15-minute driving radius of SPH. It serves patients with acute medical conditions who are clinically stable enough to be safely cared for at home.
Within the context of a client- and family-centered model of care, and guided by Providence Health Care’s Mission, Vision, Values, and strategic directions, the Registered Nurse, Hospital at Home:
Promotes a safe, respectful, and inclusive environment for patients, families, visitors, and staff
Assesses, plans, implements, and evaluates nursing care for patients with acute health needs in the home and/or hospital setting
Develops and revises individualized care plans, incorporating patient and family teaching and referrals to community resources as needed
Provides direct nursing interventions and documents care according to professional standards and PHC protocols
Collaborates with and supports colleagues by sharing clinical knowledge and demonstrating best practices
Works within an interdisciplinary clinical framework in alignment with BCCNM Standards of Practice and PHC values
Education
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
Minimum of one year of acute care nursing experience
Valid BC driver’s license and access to a personal vehicle for local travel
