Summary

Join the Kidney Care Clinic at SPH, an outpatient service dedicated to providing comprehensive care for individuals with chronic kidney disease. Our clinic focuses on preserving kidney function, managing associated conditions such as diabetes and hypertension, and preparing patients for dialysis and transplantation when needed.

We are currently seeking a Registered Nurse (RN) to join our Kidney Care Clinic—a specialized outpatient setting focused on supporting patients with chronic kidney disease (CKD) through education, monitoring, and collaborative care. 

 

As a Kidney Care Clinic Nurse, you will play a vital role within the multidisciplinary team, serving as a key coordinator, patient advocate, and primary point of contact. You will build ongoing relationships with patients through in-person clinic visits, review regular bloodwork, and manage patient concerns via phone and email.

In this role, you will provide essential education and support related to medical treatments, lifestyle adjustments for kidney health, and symptom management. This position is ideal for nurses who excel in collaborative team environments, have a strong understanding of chronic disease management, and are passionate about patient education and empowerment.

What You Bring

Education: 

  • Registered Nurse (RN) in good standing with the BC College of Nurses and Midwives (BCCNM) 
  • Completion of a recognized nephrology nursing program or equivalent experience preferred 
  • Minimum two years of recent experience in renal nursing, including one year of patient education 
  • Experience in outpatient or ambulatory care settings is an asset 

Skills & Abilities: 

  • Strong clinical assessment, communication, and teaching skills 
  • Ability to work independently and collaboratively in a team-based environment 
  • Proficiency in electronic documentation and care planning 
  • CPR certification and knowledge of CKD management best practices 
  • Commitment to continuous learning and quality improvement

Your Day to Day: 

  • Conduct comprehensive patient assessments and develop individualized care plans in collaboration with nephrologists and allied health professionals 
  • Provide patient and family education on kidney disease management, treatment options, and lifestyle modifications 
  • Monitor patient progress, adjust care plans, and coordinate follow-up care to support optimal health outcomes 
  • Collaborate with interdisciplinary teams to ensure continuity of care across the renal program 
  • Maintain accurate documentation using electronic health records and contribute to quality improvement initiatives 
  • Participate in case conferences, staff meetings, and ongoing professional development 
  • Support patients in navigating the healthcare system, including referrals to community resources and specialists 
  • Advocate for patient-centered care and uphold PHC’s values of compassion, respect, and social justice
