• Demonstrate expertise and role model specialized geriatric care skills for acutely ill seniors with complex needs
• Provide direct nursing care including assessments, medication administration, and treatments
• Complete comprehensive geriatric nursing assessments in the ED using validated tools and clinical judgment
• Monitor patient condition, identify life-threatening changes, and intervene appropriately
• Teach patients and families how to manage care, administer treatments, and use medical equipment safely
• Develop and update patient/family education materials such as brochures, manuals, and instruction sheets
• Assess readiness to learn by evaluating physical, emotional, and cognitive factors through assessments and collaboration with the care team
• Collaborate with ED and Geriatric Care Team to provide education and direction for managing seniors’ care
• Provide rapid geriatric assessments to support timely treatment, discharge planning, or admission decisions
• Coach and educate staff through role modeling, individual and group training, and acting as a clinical resource
• Facilitate discharge planning by making referrals, arranging community supports, and liaising with external agencies
• Ensure continuity of care by following up post-discharge and updating care plans in collaboration with the interdisciplinary team
• Advocate for patients and support development of health-promoting behaviours in the context of illness
• Support quality improvement by identifying trends and making program recommendations to leadership
• Contribute clinical expertise to the development of standards, policies, and educational content
• Participate in research, quality initiatives, staff development, and internal/external committees
• Maintain and update treatment records, patient documentation, and service statistics
• Re-prioritize tasks as needed based on changing patient and department needs
• Perform other related duties as assigned
