Are you a skilled Social Worker passionate about supporting older adults and their caregivers through the challenges of dementia care? We are seeking a compassionate and experienced professional to join our interdisciplinary team at the Older Adult and Dementia Caregiver Resilience Clinic.

Reporting to the Professional Practice Leader and under the direction of the Patient Care Manager, the Social Worker, Older Adult and Dementia Caregiver Resilience Clinic provides advanced therapeutic services for older adults and their caregivers in the context of dementia.

Working from a systems and structural theory-based model, the Social Worker supports a designated caseload and plays a vital role in care coordination, education, and community engagement.

 

Key Responsibilities:

  • Provide advanced clinical services, including complex assessments, individual and group therapy, and caregiver education
  • Conduct comprehensive biopsychosocial assessments and maintain accurate patient/resident records
  • Deliver therapeutic counselling to patients, families, and caregivers
  • Support and lead the interdisciplinary team in discharge planning
  • Offer consultative services to staff and community health professionals
  • Coordinate and facilitate clinical/case conferences
  • Develop and deliver in-services and educational presentations for staff
  • Participate in local committees and team meetings
  • Liaise with community agencies, hospitals, mental health services, and charitable organizations
  • Promote inclusive care practices that respect cultural, religious, and ethnic diversity
  • Stay current with developments and best practices in the Social Work profession

What You Bring

Education, Training, and Experience

  • Master’s Degree in Social Work.
  • Completion of post-master’s training in evidence-based therapeutic modalities, such as:

    • Mindfulness-Based Therapy

    • Cognitive Behavioural Therapy (CBT)

      • Or other relevant, equivalent modalities

  • Minimum of two (2) years’ experience working with individuals living with dementia and supporting their caregivers.

  • An equivalent combination of education, training, and experience may be considered.

  • Current full registration with the British Columbia College of Social Workers. (Clinical Registration is preferred)

Skills and Abilities

  • Strong knowledge of dementia care, including pathophysiology, neuropsychiatric symptoms, and therapeutic interventions such as Mindfulness-Based Dementia Care and CBT.

  • Skilled in psychotherapeutic and psychoeducational group facilitation, with the ability to communicate effectively with older adults experiencing cognitive impairment.

  • Proven ability to work independently and within inter-professional teams, with sound understanding of community resources and referral processes.

  • Committed to practicing cultural safety and humility, with the ability to educate and consult with clients, families, and healthcare professionals.

  • Able to manage a complex workload, with knowledge of healthcare regulations and proficiency in using computers and related software.

Your Day to Day

  • Participate in the development and implementation of treatment practices, guidelines and protocols within the team to promote excellence in social work practice within the Older Adult and Dementia Caregiver Resilience Clinic.
  • Participates in complex care planning with community agencies and assists in coordinating treatment follow-up with other team members and community partners through methods such as researching available services, liaising with service and program providers, advocating for services, and providing advice and consultation to colleagues.
  • Establishes and maintains effective collaborative and constructive liaison relationships with various social workers, academic institutions, hospitals and other agencies, in order to coordinate services across the continuum of mental health services.
  • Provides advanced therapeutic clinical counseling services to clients, families and groups in designated program area by exploring problems and personal concerns, treatment objectives, and emotional responses; facilitating change in coping style, behaviour, attitude and feelings; participating with clients, families, interdisciplinary team members and community partners to coordinate services, facilitate meetings and make referrals to appropriate resources. Delivers psychotherapeutic group facilitation for older adult and dementia caregiver populations.
  • Conducts comprehensive social work psychosocial assessments in designated program area using methods such as interviewing the client and/or family to obtain relevant information, gathering social data regarding the client’s family, supports, environment, culture, living situation and financial status and interpreting the data to inform interventions and plans to enhance the client’s and family’s coping abilities.
  • Formulates and implements care and treatment plans through methods such as negotiating a process for achieving goals, collaborating with members of the interprofessional team(s) in both inpatient and outpatient settings and evaluating goals while maintaining respect for client dignity, culture, self-determination and independence.
  • Maintains client records through electronic methods such as documenting client assessments, charting client information and regular progress notes and reports in accordance with established standards, policies and procedures to meet regulatory requirements and in accordance with the Freedom of Information Act.
  • Participates in program planning and development of clinical practice guidelines as part of a multi-disciplinary team. Identifies, promotes and leads program planning strategies to enhance system processes and resources for patients.
  • Takes initiative in keeping up-to-date on research and best practices in treatment methods in the older adult and dementia field and specific to social work practice.
  • Maintains knowledge of community resources locally, nationally and internationally in the field of older adults and dementia care.
  • Initiates and participates in psychosocial research. Implements appropriate evidence-based research outcomes into strategic planning and professional practice.
  • Supports patients/Families/Teams in the understanding legislative process and criteria particularly related to: Identifying Substitute Decision Makers under Health Care. Consent and Care Facility
  • Act (HCCCFAA), Requesting Medical Assistance in Dying, Providing Rights Advice under Mental Health Act, Acting as an Incapability Assessor under HCCCFAA , and acting as a Designated Responder under Adult Guardianship Act.
  • Advocates for the patient, family or caregiver to address gaps in resources within the hospital and the community. Assists patients and families with referrals to community agencies as needed
  • Provides practicum supervision and teaching to master’s level Social Work students regarding the social work role, individual and group therapy.
  • Participates in relevant committee work within the program, the hospital and the community.
  • Performs administrative tasks such as writing reports, maintaining statistics and updating health records.
  • Performs other related duties as assigned.
