We are looking for a dedicated Social Worker to join our fast-paced, compassionate team in the Emergency Department. In this role, you’ll assist patients and families in navigating urgent emotional and social challenges, ensuring they have access to the support and resources they need during their healthcare journey.

This role is an integral part of an interdisciplinary team, supporting the delivery of patient-centered care under the direction of the Patient Care Manager. Reporting to the designated Social Work Site Leader, with guidance and support from the Professional Practice Leader and/or Research, Education, and Practice Coordinator, this position focuses on providing clinical counseling, conducting biopsychosocial assessments, and supporting discharge planning. The Social Worker will work collaboratively to help achieve patient and program goals, while promoting positive outcomes for both patients and residents.

 

Education Requirements:

  • Master’s or Bachelor’s degree in Social Work is required to be considered for this position

 

Key Responsibilities:

  • Clinical Counseling & Assessment: Provide therapeutic clinical counseling services to patients and residents, conducting comprehensive biopsychosocial assessments to identify needs, strengths, and goals.

  • Interdisciplinary Team Support: Actively collaborate with the interdisciplinary program/team to provide consultative services and support discharge planning. Offer leadership within the team to ensure that patient care goals are met efficiently and effectively.

  • Consultation & Resource Connection: Provide consultation services to patients, families, staff, and community health professionals, offering guidance on available community resources and services that can assist with patient care and overall wellbeing.

  • Case Conferences & Staff Development: Coordinate clinical and case conferences, ensuring effective communication and coordination within the care team. Develop and deliver educational in-services and presentations to staff to promote best practices in social work and patient care.

  • Participation in Committees & Meetings: Participate actively in local committees and group meetings, contributing valuable insights and maintaining strong relationships across the organization. Stay informed on the latest trends and developments within the social work profession to continuously improve services and care delivery.

  • Record Keeping & Documentation: Ensure accurate and timely documentation of patient and resident records in compliance with legal, ethical, and organizational standards, ensuring continuity of care.

This role is vital in ensuring that the social work team delivers high-quality care that aligns with the mission and goals of the organization, while supporting both patients and the interdisciplinary team.

What You Bring

Education Requirements:

  • Master’s in Social Work from an accredited university, with one year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience.
  • Current full registration with the British Columbia College of Social Workers.
  • Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training, and experience.

Skills and Abilities

  • Demonstrated ability to communicate effectively with patients, residents, families, staff, physicians, and other healthcare providers.
  • Knowledge of legislation and government policies and procedures related to healthcare.
  • Demonstrated ability to gather relevant data and conduct biopsychosocial assessments.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Ability to facilitate and influence group and inter-group meetings, processes, and decision-making

Your Day to Day 

  • Therapeutic Counseling & Coordination: Provide clinical counseling to patients and families, helping to identify concerns, set treatment goals, and facilitate positive changes in behaviors. Collaborate with interdisciplinary teams to coordinate services and referrals.

  • Social Work Assessments: Conduct comprehensive psycho-social assessments by gathering relevant information from patients, families, and other sources to assess the impact on coping abilities and provide therapeutic support.

  • Discharge Planning & Resource Coordination: Contribute to discharge planning by assessing care needs and liaising with community partners to ensure effective transition and resource utilization for patients with complex needs.

  • Consultative Services & Legal Support: Offer consultative services to patients, families, and healthcare teams regarding community resources and legal processes, including rights advice and assistance with medical assistance in dying, mental health issues, and guardianship.

  • Program Evaluation & Staff Training: Evaluate program effectiveness and contribute to continuous improvement. Develop and deliver training and presentations for staff, ensuring ongoing professional development in line with social work best practices.
