Salary

The salary range for this position is CAD $49.20/Hr. - CAD $63.47/Hr.

Summary

 

Reporting to the Manager, Care Management & System Navigation, the Care Management Leader:

  • Practices according to BCCNM standards, professional ethics, and PHC’s client- and family-centered care model.
  • Coordinates patient care from admission, collaborating with the interprofessional team to ensure effective care and timely discharge planning.
  • Conducts nursing assessments, identifies barriers, and advises on appropriate care levels and resources.
  • Secures funding and approvals for required services to support optimal patient outcomes.
  • Develops and evaluates policies, procedures, and standards for care management and discharge planning.
  • Participates in committees, task groups, education, and corporate initiatives.
  • Utilizes data to guide practice, measure progress, and enhance care quality and efficiency.

What You Bring

Education

  • Baccalaureate degree in Nursing, Three (3) years recent, related experience including (1) year of experience in community care and discharge planning or an equivalent combination of education, training and experience.
  • Current registration with the British Columbia College of Nurses and Midwives.

Skills and Abilities

  • Comprehensive knowledge of Nursing theory and practice within a patient/family centred model of care.

  • Strong knowledge of nursing theory, practice, and BCCNM standards.

  • Proficient in pathophysiology, pharmacology, and discharge planning.

  • Skilled in assessing medical appropriateness and resource utilization.

  • Familiar with clinical pathways, community resources, and interdisciplinary roles.

  • Effective in patient assessment, observation, and monitoring.

  • Strong organizational, analytical, and leadership skills.

  • Excellent verbal and written communication.

  • Proficient in computerized care systems.

  • Able to adapt, resolve conflicts, and make decisions.

  • Physically capable of performing job duties.

Your Day to Day: 

  • Lead care coordination and discharge planning with the interprofessional team, ensuring timely and safe transitions.
  • Monitor patient progress, adjust care plans, and address barriers to optimal outcomes.
  • Facilitate daily care rounds and assign tasks to support discharge goals.
  • Act as a resource for referrals, post-discharge care, and funding approvals.
  • Collaborate with teams, physicians, and external providers to resolve care issues and improve flow.
  • Advocate for patients’ rights, dignity, and access to information.
  • Maintain accurate patient records and support process improvements.
  • Train and mentor staff while staying updated on clinical best practices.
  • Participate in initiatives, policy development, and professional growth.
