Social Worker - Road to Recovery Program at Providence Health Care

Are you a Social Worker passionate about making a real impact on individuals facing substance use and mental health challenges? Providence Health Care is hiring for the Road to Recovery Program in a Regular Full-Time position

Join our innovative, interdisciplinary team providing holistic, person-centred, trauma-informed care. With the new stabilization unit opened in Fall 2023 and continued program expansion, you’ll be part of a cutting-edge approach to recovery. Our program offers detox beds, transitional care,  aftercare outreach services, ensuring clients receive the right support at every stage of their journey.

In alignment with the Mission, Vision, Values, and strategic directions of Providence Health Care, the person in this role will promote a safe, respectful, and civil working environment for patients, residents, families, visitors, and staff.

Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator, the position supports the interdisciplinary program/team, which is under the direction of the Patient Care Manager, in achieving program, patient and resident goals.

 

Key Responsibilities:

  • Therapeutic Support: Provide clinical counselling services and conduct comprehensive biopsychosocial assessments to help individuals achieve their recovery goals.
  • Collaboration & Leadership: Work closely with an interdisciplinary team, supporting discharge planning, providing consultative services, and guiding team members in best practices.
  • Ongoing Development: Coordinate clinical case conferences, deliver staff training, and actively participate in local committees to advance the Social Work profession.
  • Community Connection: Serve as a resource to patients, families, staff, and community health professionals, offering guidance on community resources

What You Bring

Qualifications:

  • Master’s in Social Work from an accredited university
  • One year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience
  • Current full registration with the British Columbia College of Social Workers
  • Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training and experience may be considered. Current full registration with the British Columbia College of Social Workers.

Skills and Abilities

  • Demonstrated ability to communicate effectively with patients/residents, families, staff, physicians, and other healthcare providers
  • Knowledge of legislation and government policies related to healthcare
  • Ability to gather relevant data and conduct biopsychosocial assessments
  • Experience in conducting patient/resident assessments
  • Knowledge of other healthcare disciplines and their roles in patient/resident care
  • Ability to draw out information to understand the patient's/resident's situation and explore issues beyond initial concerns
  • Proven ability to work collaboratively as a member of an interdisciplinary team
  • Skill in conflict resolution, including guiding others through resolution issues
  • Physical ability to perform the duties of the position
  • Strong independent work skills, with the ability to prioritize tasks and workload
  • Proficient in operating related equipment
  • Willingness and ability to participate in research
  • Experience in providing supervision and teaching to students
  • Ability to facilitate and influence group and inter-group meetings, processes, and decision-making

Your Day to Day 

  • Therapeutic Counselling Services: Provides clinical counselling to patients, families, and groups, exploring client concerns, facilitating behavioral and emotional changes, and coordinating with interdisciplinary teams and community resources.
  • Social Work and Psycho-social Assessments: Conducts comprehensive assessments, gathering data on family, environment, culture, and financial status to inform therapeutic interventions and enhance coping abilities.
  • Record Maintenance: Documents assessments, progress notes, and reports in compliance with policies and regulatory requirements.
  • Discharge Planning: Contributes to discharge planning by assessing patient needs, coordinating with community partners, and ensuring effective use of resources for patients with complex needs.
  • Consultative Services: Provides guidance on community resources, financial assistance, and access to services to optimize planning for patients and families.
  • Interdisciplinary Collaboration: Participates in clinical care conferences and contributes to team discussions on patient admissions, reviews, and discharges.
  • Legislative Support: Assists patients and families in understanding legal processes related to healthcare decisions, such as medical assistance in dying, substitute decision-making, and mental health act rights.
  • Program Evaluation: Offers social work perspective in evaluating patient care and program effectiveness, ensuring appropriate treatment.
  • Training and Development: Develops and delivers in-services, presentations, and training for staff, collaborating with interdisciplinary team members.
  • Committee Participation: Engages in program evaluations and identifies opportunities for improvement in service delivery to enhance patient care and reducilitation: Facilitates group sessions, offering counseling and psycho-social education to patients and families.
  • Professional Development: Stays updated on social work practices through literature, expert consultations, and professional development activities.
  • Research and Data Analysis: Collects and analyzes workload data, contributes to research, and ensures clinical practices align with current findings.
  • Supervision and Teaching: Provides guidance and supervision to social work students, fostering their development through practical experience.
  • Additional Duties: Performs other related tasks as required.
