Contribute to Transformative Care as a Social Worker in our Kidney Transplant Program at St. Paul’s Hospital
St. Paul’s Hospital, one of BC’s two adult kidney transplant centers, is seeking a dedicated and compassionate Social Worker to join our dynamic Kidney Transplant Program. This comprehensive program includes transplant surgery, pre- and post-transplant care, and living donor programs. As part of the interdisciplinary team, the Social Worker will play a vital role in the holistic assessment of patients being considered for kidney transplants. The role involves assessing patients’ psychosocial needs and providing the necessary support to ensure they are suitable candidates for transplantation.
The Kidney Transplant Program at St. Paul’s works closely with a network of key partners, including BC Transplant, the BC Renal Agency, Canadian Blood Services, and the Provincial Health Services Authority. You will have the unique opportunity to be part of a life-saving treatment program, collaborating with nephrologists, transplant surgeons, nurses, dietitians, and other healthcare professionals.
Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research, Education, and Practice Coordinator, this position plays a key role in supporting the interdisciplinary program/team under the direction of the Patient Care Manager. The primary focus is to help achieve program, patient, and resident goals by providing clinical support, coordination, and leadership. Key duties include therapeutic counselling, assessments, consultation, and contributing to team processes in the discharge planning and care delivery.
