Providence Health Care (PHC) is hiring a Resident Care Manager for Long-Term Care at Brock Fahrni, a vibrant long-term care facility housing 130 residents. Position Overview:

 

As the Resident Care Manager, you will report to the Director, Operations or Director, Clinical Operations, Seniors Care. This key leadership role involves providing operational leadership and support to interdisciplinary team members in delivering high-quality care to residents and their families.

 

Key Responsibilities:

  • Collaborate with a Physician Leader to ensure the effective and efficient delivery of services within designated sub-units of a large clinical program.
  • Lead and supervise interdisciplinary staff, including Clinical Nurse Leaders (CNLs) and/or Unit Supervisors or equivalent nursing leadership roles.
  • Promote and facilitate the continuum of care delivery, working closely with internal and external agencies, and aligning with the program goals and strategic direction.
  • Utilize quality improvement, utilization management, staff development, and resource management strategies to enhance care delivery.
  • Work within the Program Based Care Model alongside Program Leaders, other Patient/Resident Care Managers, Practice Leaders, and Corporate and Support Leaders.
  • Act as Site Leader for multiple sites or programs (e.g., Youville/Brock Fahrni, Langara, Holy Family), coordinating with clinical, corporate, and support leaders to ensure smooth operations and consistent application of corporate policies and regulations.

Qualifications :

The following qualifications and competencies are required to successfully provide the leadership necessary to fulfill the Mission, Vision, and Values of Providence Health Care:

  • Masters degree in a related Health Profession
  • Minimum of five (5) years' experience at the middle management level in a medium to large health care organization
  • Or an equivalent combination of education, training, and experience

What We Offer

1. Provides leadership to the interdisciplinary team including CNL(s) and/or Unit Supervisor(s) and/or equivalent nursing leadership roles to ensure patient /resident/family focused care is delivered within a coordinated and integrated care delivery model.

2. Leads agreed to change initiatives at the site by participating in the planning and development of activities on behalf on the site; chairs the site council and other groups and interfaces/links with community groups, foundation and internal stakeholders to ensure a continuum of communication is provided and site concerns are advanced.

3. Works in collaboration with other leaders to gather information and assist in cross-program and/or program -support service problem-solving and communication for the site.

4. Collaborates with the physician operations leader to plan and manage the provision of effective and efficient care within assigned sub-units to support the achievement of operational and strategic objectives of the clinical program and consistent with the mission, vision and values of PHC.

5. Collaborates with other leaders to lead and/or participate in process improvements by engaging the interdisciplinary team in problem-solving and applying tools of quality improvement, utilization and risk management including implementation of standards and tracking mechanisms and use of data and variance analysis.

6. Works with community and/or internal partners as appropriate to ensure the provision of timely care to meet the needs of patients/residents/families. Assists teams to manage risks, incidents, family/client complaints and discharge planning.

7. Ensures effective, two-way communications with and among team members so that each team is aware of and understands changes impacting them, the site, the organization and how such changes will impact delivery of services.

8. Provides the interface between the assigned sub-units and corporate /support systems to identify needs and participate in identifying solutions that will enable the front-line delivery of care.

9. Collaborates with physician partners to develop the operating and capital budget for the areas of responsibility, monitor productivity indicators, budget variances and analysis. Approves timekeeping records, authorizes overtime and compiles periodic operational reports.

10. Develops, recommends, and implements, in collaboration with Human Resource Representative(s), strategies for recruitment and retention of staff that support the goals and objectives of the clinical area(s) of responsibility.

11. Facilitates the development and implementation of orientation programs for new staff to provide the appropriate education in clinical practice routines, and hospital policies and procedures.

12. Develops, recommends, implements, and evaluates staffing patterns, rotations and vacation schedules to ensure sufficient staff to meet patient/resident care needs.

13. Participates as a member of the leadership team by performing related duties such as formulating employer demands for collective bargaining, developing strike contingency plans, representing the employer during grievance proceedings, participating in the managerial 24 hour call structure and facilitating and initiating service delivery change proposals.

14. Creates an environment that embraces and facilitates interdisciplinary team learning, evaluation and continuous improvement.

15. In collaboration with Professional Practice Leaders:
•Ensures that team practices align with established clinical/corporate standards, guidelines, systems.
•Determines skill mix and staff complement required to support clinical care in assigned sub-units; selects staff; determines performance expectations and provides coaching based on the goals and objectives of the clinical areas of responsibility.
•Facilitates the development and implementation of orientation programs for new staff to provide the appropriate education in clinical practice routines and hospital policies and procedures.
•Develops, recommends, implements and evaluates staffing patterns, rotations and vacation schedules to ensure staffing to meet patient/resident care needs.
•Jointly conducts performance appraisals of interdisciplinary care teams and their members.
•Ensures fieldwork opportunity for interdisciplinary students including orientation to the work place and evaluation /feedback of performance.
•Fosters the concept of evidence-based practice.

16. Performs other related functions as assigned.
