About Hospital at Home

The Hospital at Home program is an innovative healthcare initiative that allows eligible patients to receive hospital-level, acute care in the comfort of their own homes. Supported by a multidisciplinary team of physicians, registered nurses, occupational therapists, physiotherapists, and social workers, we ensure comprehensive treatment through:

  • Daily in-person visits

Your Role

  • Reports to the Operations Leader and coordinates the day-to-day operations of a designated nursing unit.
  • Supervises nursing staff and serves as a clinical resource to nursing and interdisciplinary team members in providing direct care to patients.
  • Assists the Operations Leader in assessing the clinical development needs and interests of staff.
  • Supports the orientation and educational programs for new staff.
  • Plays a key role in promoting research and quality improvement initiatives within the program team.

What You Bring

Qualifcations

  • Registration with the British Columbia College of Nurses and Midwives as a practicing RN registrant.
  • Qualifications required for staff nurses in the clinical area.
  • Three years of recent, related experience in the clinical specialty.
  • Alternatively, an equivalent combination of education, training, and experience may be considered.

**For Clinical Nurse Leader vacancies in mental health services and in extended and intermediate care services, positions will be dual posted for Registered Nurse/Registered Psychiatric Nurse with current practicing registration with the British Columbia College of Nurses and Midwives as a practicing RN or a practicing RPN registrant.

  • Coordinates day-to-day patient/resident care: Oversees nursing unit operations by supervising staff, actively monitoring and following up on their work, coordinating assignments, providing input on employee performance, orienting new employees, calling in relief staff as needed, and authorizing overtime shifts. Troubleshoots staffing issues and problem-solves when needed.

  • Monitors patient/resident acuity and flow: Ensures staffing levels align with patient/resident needs and unit requirements by adjusting assignments as acuity levels change.

  • Acts as a clinical resource: Provides support to nursing staff, students, and other interdisciplinary team members by demonstrating procedures, addressing clinical practice issues, problem-solving, and troubleshooting concerns. Advises the Operations Leader or other resource persons on issues or concerns.

  • Participates in rounds/clinics: Collaborates with nursing staff to assess patient/resident needs, prioritize nursing interventions, and evaluate interventions.

  • Coordinates consultations: Works with nursing staff to identify when consultations with physicians, nurse consultants, or other disciplines are necessary and arranges for these consultations.

  • Advocates for patients/residents and families: Communicates patient/resident and family concerns to the care team and intervenes when required.

  • Performs direct nursing care: Takes responsibility for a patient/resident assignment and provides necessary nursing care.

  • Provides input on orientation and educational programs: Collaborates with the healthcare team to develop, evaluate, and deliver orientation programs for new employees. Makes recommendations for changes or new initiatives.

  • Identifies and addresses learning needs: Works with the Operations Leader and team members to identify learning needs of nurses and implements educational strategies. Presents in-service educational sessions as required.

  • Reviews policies and procedures: Provides feedback and input to the Operations Leader regarding changes or modifications to policies, procedures, and reference materials, updating them as approved.

  • Mentors and assists with education: Helps promote further education by mentoring staff on educational opportunities, assisting with conflict resolution, and developing clinical nursing skills.

  • Assists with Performance Planning and Coaching: Provides input to the Operations Leader regarding staff performance and acts as a clinical resource during the Performance Planning and Coaching process.

  • Supports research initiatives: Helps identify clinical care problems that can be researched, participates in related research projects, and assists with implementing research findings into practice.

  • Provides input on equipment, supplies, and staffing: Offers recommendations to the Operations Leader on the need for equipment, supplies, and staffing, addressing concerns and proposing solutions to problems.

  • Collaborates on quality improvement initiatives: Works with the interdisciplinary team to recommend and implement quality improvement projects, evaluates results, and makes adjustments as needed, with authorization from the Operations Leader.

  • Performs other related duties: Carries out additional tasks as assigned.
