Join Our Compassionate Team at St. Paul’s Hospital

We are seeking a dedicated and compassionate Social Worker to join our dynamic team in the Emergency Department at St. Paul’s Hospital. In this role, you will help local community members navigate emergent and urgent social/emotional crises related to acute health needs that bring them to the hospital, in a fast-paced, dynamic healthcare environment. You will support patients and families in navigating crises, accessing primary care, social services, benefits, and community supports. Additionally, you will provide critical support to patients and families at various points in their healthcare journeys, starting from their initial presentation at the hospital.

Report to the designated Social Work Site Leader and receive guidance from the Professional Practice Leader and/or Research, Education, and Practice Coordinator. Your role includes:

• Supporting the interdisciplinary team in achieving patient and resident goals under the direction of the Patient Care Manager.
• Providing therapeutic clinical counseling services to patients and residents.
• Conducting comprehensive biopsychosocial assessments to evaluate needs.
• Maintaining accurate patient/resident records to ensure continuity of care.
• Providing consultative and direct services regarding community resources to patients, families, staff, and community professionals.
• Leading discharge planning with the interdisciplinary team, ensuring smooth transitions for patients and residents.
• Coordinating clinical/case conferences, ensuring effective communication and decision-making regarding patient care.
• Developing and delivering in-service training for staff to enhance team capabilities and knowledge.
• Participating in committees and meetings, contributing to program development and improvement.
• Staying informed about advancements in the Social Work profession, ensuring up-to-date practices and knowledge.

Education

• Master’s in Social Work from an accredited university, with one year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience.
• Current full registration with the British Columbia College of Social Workers.
• A Bachelor of Social Work degree from an accredited School of Social Work, along with an equivalent combination of education, training, and experience, may also be considered.

• Ability to participate in research and contribute to teaching and supervision of students.

Your Day to Day

• Provides therapeutic clinical counseling to patients, families, and groups, identifying problems and treatment objectives while facilitating behavioral and emotional changes.
• Conducts comprehensive social work and psycho-social assessments, gathering data on family, environment, culture, and financial status to support patient coping abilities.
• Maintains patient records, ensuring proper documentation of assessments, progress notes, and reports in accordance with regulatory standards.
• Contributes to discharge planning by assessing care needs, coordinating services with community partners, and ensuring optimal use of resources for patients with complex needs.
• Offers consultative services regarding community resources and financial assistance, helping patients and families navigate available support systems.
• Participates in interdisciplinary team conferences for patient reviews, admissions, and discharges, contributing to treatment plans and decisions.
• Supports patients and families in understanding legislative processes and criteria, including health care consent, medical assistance in dying, and mental health rights.
• Provides ongoing program evaluation, offering insights on patient care issues and program effectiveness to improve treatment.
• Develops and delivers in-service training, presentations, and seminars for staff, ensuring effective interdisciplinary collaboration.
• Supervises social work students, providing guidance and practical experience in treating patients and residents.
