The salary range for this position is CAD $49.20/Hr. - CAD $63.47/Hr.

Summary

Providence Health Care's Eating Disorders Program is currently accepting applications for a temporary part-time (0.6 FTE) Clinical Nurse Leader, Central Intake Vacation Relief to join their team. Our RNs and RPNs are core members of our interdisciplinary care teams, working closely with other disciplines like Social Work, Physiotherapy, and Occupational Therapy to tackle the variety of treatment needs of the patient populations we serve. Please note this position is currently scheduled to last until November 21st 2025 or the return of the incumbent. 

Reporting to the Operations Leader, the Clinical Nurse Leader/Central Intake coordinates the day-to-day operations of the designated unit and supervises staff. Acts as a clinical resource to nursing staff and interdisciplinary team members in the provision of direct care to patients. Assists the Operations Leader in assessing clinical development needs/interests of staff, and with orientation and educational programs. The CNL/Central Intake also has primary responsibility for coordinating referrals to Providence Health Care's (PHC) Provincial Outpatient Psychiatry Program. Triages provincial referrals to the appropriate level of care within PHC's program or to the appropriate nearest community agency. Expedites an action plan for referred patients to ensure timely and appropriate access to necessary care. Promotes research and quality improvement initiatives within the program team.

What You Bring

Education

  • Baccalaureate Degree in Nursing or Psychiatric Nursing, plus three (3) years of recent related experience in mental health including two years experience in outpatient psychiatry or an equivalent combination of education, training and experience.
  • Current registration with the British Columbia College of Nurses and Midwives.

Your Day to Day

  • Coordinates the day to day patient care activities of the unit by performing duties such as supervising staff, the active and ongoing monitoring and follow-up on the work of staff, coordinating work assignments, providing critical input on employee performance, providing input into the development, evaluation, and delivery of orientation, in conjunction with other members of the interdisciplinary health care team and orientating new employees, calling in relief staff as necessary, and authorizing overtime shifts. Problem-solves and troubleshoots staffing issues. Monitors the patient acuity and flow of patients on the unit, to ensure that staffing levels match patient and unit needs. Makes adjustments to the assignments as changes in acuity occur.
  • Collaborates with the Operations Leader the nursing staff, and other team members, in identifying specific learning needs of nurses and other interdisciplinary team members and implementing educational strategies and programs. Presents in-service educational sessions for staff as required. Assists the Operations Leader and other resource nurses in promoting further education, by mentoring staff regarding educational opportunities, and assisting with interpersonal conflict resolution and the development of clinical skills.
  • Acts as a clinical resource to nursing staff, students, and other members of the interdisciplinary team, by demonstrating procedures, answering questions related to clinical practice issues, coaching, problem solving, and troubleshooting concerns. Advises Operations Leader and /or resource person of issues/concerns.
  • Collaborates with nursing staff to determine needs for consultation with Physicians, Nurse Consultants, and other disciplines or departments. Arranges for consultation when required.
  • Participates in rounds/clinics and collaborates with nursing staff by assisting in the assessment of patient problems and needs, planning and prioritizing of nursing intervention, and implementing and evaluating interventions.
  • Reviews policies, procedures, and reference materials for nursing and other multi-disciplinary team members, and provides feedback and input to the Operations Leader or designate, for changes and modifications. Updates and communicates changes to staff as approved. Identifies trends in patient outcomes and recommends program modifications to the Operations Leader.
  • Triages referrals to the appropriate service through methods such as:
    • receiving and assessing referrals and determining appropriate program/service;
    • referring patients and families to the appropriate community resource if patient's needs are better served elsewhere;
    • acting as a liaison between the Physician Leaders and/or Internal Medicine if suitability for the program or services is not clear;
    • following-up with the referral source to ensure the reason for referral is clear and that there is sufficient information to select the appropriate program or service;
    • arranging for/providing assessment if insufficient information or the urgency of the referral is unknown.
  • Assists the Operations Leader, Physician Leader(s) and Professional Practice Leader, Psychology in the development and monitoring of the Central Intake Service. Duties include collaborating with designated interdisciplinary team members of all services within PHC's system as well as patients to ensure a smooth transition from home to PHC based services. Consults and follows up with PHC physicians and staff to ensure patients are seen in a timely fashion. Problem solves with staff and physicians in the individual services to prioritize/re-prioritize referrals and reduce wait lists.
  • Maintains a provincial wait list for designated programs and services. Duties include receiving inquiries in conjunction with other interdisciplinary team members, their availability, admission criteria for the various programs and services, how to access programs and respond or delegate as necessary.
  • Provides the referral source with an action plan for care at the end of the referral or assessment. Provides feedback and updates on status of patient referral to referral source.
  • Provides direct nursing care through methods such as:
    • Assessing patients and/or coordinating the assessment of physical, psychosocial and environmental problems;
    • Analyzing patient needs and compiling a related care plan;
    • Identifying and coordinates other disciplines such as specialists or general practitioners to provide outreach or outpatient visits who may need more immediate assessment;
    • Co-leading patient follow-up group(s) with the designated psychologist;
    • Leading Provincial Triage rounds for PHC's outpatient psychiatric programs and services.
  • Acts as the patient and family advocate by communicating patient and family concerns to other members of the health care team. Intervenes if required.
  • Liaises with regional and community partners and leads the formulation, interpretation, implementation and evaluation of central intake system and processes to ensure timely and appropriate access to PHC's outpatient psychiatric services.
  • Identifies and participates in meeting the learning needs of community health professionals, patients and family members in relation to the program through methods such as providing education and consulting on treatment decisions for patients at regional and community meetings.
  • Represents the program/service on hospital and community committees as requested, maintaining liaisons with external agencies. Reports back to program/service.
  • Assists the Nurses and other members of the interdisciplinary team in the identification of researchable clinical patient care management problems. Participates in research initiatives and projects related to patient care and/or nursing practices as requested, disseminates findings, and assists in applying and implementing research findings into practice.
  • Provides input and recommendations to the Operations Leader regarding equipment, supplies, and staffing requirements. Advises the Operations Leader of concerns and problems and provides suggestions to resolve problems.
  • Collaborates with interdisciplinary care team members regarding quality improvement initiatives, by making recommendations for QI projects, implementing processes and procedures upon authorization, and evaluating results. Makes adjustments as required, upon authorization of Operations Leader.
  • Performs other related duties as assigned.
