Salary

The salary range for this position is CAD $74.45/Hr. - CAD $81.90/Hr.

Summary

Owned and operated by Providence Health Care, the Foundry Granville Clinic provides physical & sexual health, mental health, substance use, peer support and social services via a multi-disciplinary team to young people between the ages of 12 and 24.The Nurse Practitioner at the Foundry Granville Clinic provides episodic, enhanced/specialized and longitudinal care. The collaborative team includes physicians, nurse practitioners, registered nurses, social workers, counsellors, occupational/rehab services, peer support, and administrative staff. At the Foundry Granville Clinic, Nurse Practitioners also have access to shared care consultation with the psychiatrists on site. Your responsibilities will include:

  • Collaborating with patients and healthcare professionals to identify trends and patterns affecting individuals, families, and communities.
  • Developing and implementing population and evidence-based strategies to improve health outcomes.
  • Participating in policy-making activities influencing health services and practices.
  • Engaging in peer review and self-review to evaluate service outcomes at patient, community, and population levels.

What You Bring

Education:

  • Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals.
  • Successful completion or awaiting results of the Objective Structured Clinical Examination (OSCE).
  • Completion of the Basic Cardiac Life Support Course, Level C.

Skills and Abilities:

  • Communicate and collaborate effectively with patients/clients and families regarding health findings, diagnosis, treatment, self-care, and prognosis.
  • Collaborate, consult with, and refer patients/clients to physicians and other healthcare professionals when necessary.
  • Critically assess and evaluate health research literature to determine best practices and introduce education based on evidence-based research.
  • Assess and recognize population health trends and plan and implement strategies for population-based prevention and health promotion.
  • Implement and evaluate planned changes in healthcare practices.
  • Supervise and evaluate the care delivered by others.

  • A competitive salary: $74.45-$81.90 per hour, depending on NP experience. Reimbursement is also available upon successful completion of the Objective Structured Clinical Examination (OSCE) for NPs who have not yet completed the exam.

Your Day to Day:

  • Diagnoses and treats patients for undifferentiated diseases, disorders, and conditions within scope of practice; provides first-line care in emergencies.
  • Monitors ongoing care, orders appropriate diagnostic investigations, interprets reports, and analyzes information to plan treatment.
  • Establishes priorities for health management; provides follow-up treatment; communicates findings, diagnoses, and priorities to patients and families; supports and counsels patients in their responses to diseases and conditions.
  • Collaborates with medical staff colleagues and healthcare team to determine appropriate resources and develop treatment plans; may admit and discharge patients according to medical staff bylaws.
  • Prescribes drugs within statutory and regulatory standards for Nurse Practitioners and PHC medical staff rules.
  • Provides mentorship and education related to patient care to registered nurses, particularly junior RNs and LPNs.
  • Implements screening and health promotion activities for at-risk populations, patients, and families.
  • Completes Encounter Records as required by the Ministry of Health to evaluate Nurse Practitioner implementation.
