- Program and Scope:
- Medicine: Adults and Older adults with chronic and acute ailments such as Upper and Lower GI Bleeds, Cerebrovascular Accidents, Coronary Artery Disease, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease
- Acute Geriatric Medicine: Older adults with acute and chronic ailments that include geriatric syndromes such as Major Neurocognitive Disorders, and Parkinson Disease, as well advanced stages of chronic diseases in the liver, kidney, cardiac, or musculoskeletal issues.
Your Day to Day:
- Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers.
- Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs.
- Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care.
- Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results.
- Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems.
- Identify and address patient/resident and family learning needs, providing education and referrals as necessary.
- Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care.
- Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures.
- Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources.
- Perform additional duties as assigned to support the healthcare team.
