Summary

Our long-term care facilities at PHC

Providence Health Care operates six Long-term Care Homes and one Assisted Living Care Home for seniors, providing 24-hour nursing care and a broad range of medical and personal assistance to support social/leisure needs and activities of daily living. The homes are located throughout the lower mainland with staff working 8-hour shifts.

 

We are hiring for the following location:

  • Holy Family Hospital at 7801 Argyle Dr, Vancouver, BC

As a Staff Nurse, you'll be at the forefront of patient care, working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support. Your responsibilities will include:

  • Assess patients' physical, social, spiritual, and psychological needs
  • Develop, implement, and evaluate individualized care plans
  • Incorporate teaching into care plans to educate patients and families
  • Make referrals to resources as needed
  • Support fellow healthcare professionals
  • Operate within the framework of Providence Health Care's mission, vision, and values

What You Bring

Education:

  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives

Skills and Abilities:

  • Demonstrated competence in performing CPR procedures
  • Experience in teamwork, decision-making and conflict resolution
  • Demonstrated a teaching aptitude
  • Proficient in clinical leadership
  • A strong desire to expand your professional skills and a commitment to lifelong learning in the nursing field

Your Day to Day:

  • Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers
  • Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs
  • Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care
  • Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results
  • Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems
  • Identify and address patient/resident and family learning needs, providing education and referrals as necessary
  • Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care
  • Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures
  • Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources
  • Perform additional duties as assigned to support the healthcare team
