- Program and Scope: Holy Family Hospital Rehabilitation Unit (HFH Rehab) is one of the largest rehabilitation referral centres in BC. The unit has 65 inpatient beds dedicated to supporting the specialized rehabilitation needs of older adults following a life-altering event, such as a stroke, other neurological issues, orthopedic trauma, as well as major surgeries like hip replacements and leg amputations. In addition, more than 15,000 patients visit HFH Rehab each year to receive outpatient rehabilitation services for needs resulting from illness or injury
Your Day to Day
- Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
- Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
- Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
- Administer medications per protocol, monitor effects, and maintain communication with the team
- Utilize evidence-based resources to stay updated on nursing practices
- Respond to incidents such as codes, injuries, spills, and alarms, following procedures
- Assist with staff orientation, demonstrating procedures and providing support
- Address patient/resident and family learning needs, provide educational materials, and make referrals
- Attend care planning meetings, provide input, and advocate for patients/residents
- Collaborate with the team to develop and evaluate individualized care plans
- Assist with discharge planning based on patient status and external supports
- Operate and clean equipment, report malfunctions, and handle hazardous materials. Perform clerical duties such as answering phones, faxing, and maintaining records
